Understanding Retinal Disorders: Breaks, Detachments, and Vitreous Dynamics

Retinal breaks are full-thickness defects in the neurosensory retina (NSR). Most retinal detachments (RDs) stem from these breaks. A common culprit is age-related changes in the vitreous humor (VH), the gel-like substance filling the eye.

As we age, the vitreous becomes more liquid, increasing its movement. This movement can cause the vitreous to pull away from the retina, a phenomenon known as Posterior Vitreous Detachment (PVD). This pulling can lead to retinal holes or tears, which are then called rhegmatogenous retinal detachments when vitreous fluid passes through the break and lifts the retina.

Vitreous Humor: Anatomy and Adhesions

The vitreous humor is over 98% water, with a gel consistency maintained by hyaluronic acid (HA) and collagen fibers. HA stabilizes the collagen fibers. Normal vitreous adhesions are strongest at the vitreous base (straddling the ora serrata), followed by the optic nerve head (ONH) and fovea/macula. Peripheral blood vessels and retina also have adhesions.

Abnormal vitreous adhesions can occur where the VH attaches to weak or abnormal retinal areas. This includes vitreous base abnormalities, pigment clumps (RPE hyperplasia), chorioretinal scars, and peripheral retinal thinning degenerations like lattice degeneration. Lattice degeneration is common in myopic individuals and characterized by strong vitreous adhesion.

Vitreoretinal Traction and PVD Progression

Vitreoretinal traction (VRT) occurs when the vitreous attaches to and pulls on the retina. Dynamic VRT, influenced by eye movement, is often responsible for rhegmatogenous RDs. Static VRT, unchanging with eye movement, is also a factor. The process leading to PVD involves vitreous liquefaction (synchysis) and vitreous syneresis (movement of the liquid VH).

PVD can be complete or partial (incomplete). Incomplete PVD, where the vitreous detaches from some areas but remains attached to others (macula, vessels, peripheral retina), is more dangerous due to the potential for traction. Acute PVD, typically presenting with sudden floaters and flashes, carries a critical 6-week period for potential retinal break formation. Chronic PVD has a lower risk of acute complications.

Symptoms of PVD include floaters (collapsing collagen fibers) and photopsia (flashes from VRT). These are more common in individuals over 40, high myopes (10-18D), and pseudophakic or aphakic patients (post-cataract surgery).

Types of Retinal Breaks and Their Risk

Retinal breaks are classified by their appearance, age, and associated traction:

  • Non-atrophic (fresh) vs. Atrophic (old) holes: Fresh holes are typically bright red; atrophic holes are deeper red. Traction is often present with fresh breaks, causing flashes.
  • Operculated holes or tears: A piece of retina (operculum) is pulled off by the vitreous, which then floats in the VH, indicating traction has largely released.
  • Fresh round holes: Vary in size, with or without surrounding fluid (cuff).
  • Flap or horseshoe tears: U-shaped tears with the opening towards the periphery and apex towards the posterior pole, often associated with traction and subretinal fluid.
  • Giant retinal tears: Large tears extending 90 degrees or more.
