Fulminant Myocarditis: A Critical Condition

A small number of patients experience fulminant myocarditis, a severe and rapidly progressing form. This can escalate within hours from a severe febrile respiratory syndrome to cardiogenic shock, potentially leading to multi-organ failure (renal, hepatic) and coagulopathy. These patients are typically young adults who might initially be discharged from urgent care, only to return quickly in critical condition.

Early recognition of fulminant myocarditis is vital because aggressive support, including high-dose intravenous catecholamines and temporary mechanical circulatory support, can be life-saving. More than half of these patients can survive, often recovering near-normal ejection fraction, although some may have residual diastolic dysfunction.

Chronic Myocarditis and Its Challenges

Chronic viral myocarditis is often suspected but rarely definitively proven, especially when no other cause for dilated cardiomyopathy (DCM) is identified. Many cases initially attributed to viral infection are later found to be due to genetic factors, excessive alcohol consumption, or illicit stimulant drugs.
